Groups of 5 delegates are eligible for the Early Bird registration rate.

Download the official Group Registration Excel Template and complete all required delegate information.

Please ensure all delegate details are accurate before submission.

Submit Your Registration

After completing the Excel template, email the completed file to the contact@fmsconference.my.

Please ensure the completed Group Registration Template is attached before sending your email.

Invoice Issuance

Once your submission has been reviewed and verified, the FMSC Secretariat will issue an Invoice within 30 days.

Payment & Confirmation

Upon receipt of payment, the FMSC Secretariat will:

  • Issue an Official Receipt.
  • Send a Registration Confirmation Email.
  • Confirm all registered delegates.

Important Notes

  • Please submit only the latest version of the registration template.
  • Ensure every delegate's email address is correct.
  • Incomplete submissions may delay invoice processing.
  • Registration is only confirmed after payment has been received.
